Helping you move from hospital to home

At MMH, our transitional care services are designed for patients recovering from an illness or surgery who no longer need hospital-level care but are not ready to go home. Our program is called transitional care because its focus is to help you or your loved one transition to the highest level of independence. Transitional care services can also be known as swing bed programs.

Because transitional care is provided within our inpatient unit, our patients benefit from having convenient access to our lab and radiology services, as well as our comprehensive care team.

Together, our caregivers create a personalized plan unique to each patient’s needs. Additionally, the team holds regular bedside rounds with patients and their families to ensure everyone understands the plan of care. Length of stay is determined by patient progress.

Transitional care services

  • Continuous physical, occupational or speech therapy

  • IV antibiotics to treat a variety of infections

  • Care for fractures or following orthopaedic surgery
  • Care for respiratory conditions due to heart failure, COPD, pneumonia and other chronic conditions

  • Specialized treatment for wounds that won’t heal
